| 01 | Duly filled in typed Power of Attorney mentioning CNIC/POC numbers and residential addresses of Executant and Attorney with two witnesses (Attach CNIC/POC and passport copies of Attorney as well), signatures of 2 witnesses with their CNIC copies. If POA consists of more than one page, each page must be signed by the Executant. |
| 02 | 2x recent photographs (3 X 4 size light blue background). |
| 03 | The applicant/s has/have to come in person to the Embassy and sign or put a thumb impression on the POA and the register of the POA. |
| 04 | Original valid Pakistani Passport, CNIC, Permit of Stay (visa) in Indonesia along with photocopies. |
| 05 | Complete address, CNIC Numbers, passport numbers, and contact numbers of the Executant and Attorney both. A data form can be taken from Embassy and filled in and submitted along with POA |
